PH Consulate to hold outreach on Tinian, Saipan

The Philippine Consulate General in Agana, Guam, in cooperation with the United Filipinos Association on Tinian, will conduct mobile consular services on Tinian, from May 9 to 10, 2014, at the Tinian Room of the Tinian Dynasty Hotel & Casino.

The consulate will also conduct similar mobile consular services on Saipan, this time with the help of the United Filipino Organizations, from May 12 to 15, 2014, at the Daidai Social Hall of the Mount Carmel Cathedral.

All interested to avail of consular services are advised to prepare the following requirements:

A. FOR PASSPORT APPLICATIONS

* Requirements for renewal of green Philippine passport:

1. Accomplished ePassport Application form

2. Photocopy of data page of passport

3. Original passport

4. Payment of ePassport Fee ($60)

* Requirements for renewal of brown/maroon Philippine passport

1. Accomplished ePassport Application form

2. Photocopy of data page of passport

3. Photocopy of birth certificate

4. Original passport

5. Payment of ePassport Fee ($60)

In order to facilitate express processing of ePassport applications during the mobile consular service, passport applicants are requested to send clear scanned copies (PDF format only) of these requirements (i.e., items 1 and 2 for green passport holders and items 1, 2, and 3 for holders of brown/maroon passports) to saipan.pcgagana@gmail.com until Friday, May 2, 2014. Applicants are advised to ensure that all items in the application form are answered fully and completely and that contact numbers are provided therein, as they will be informed of their schedule for biometrics capturing. Due to technical capacity of our equipment, we will only accept the first 480 completed applications for the “pre-encoded applicants.”

It is important that applicants should submit completely the required documents as the consulate general will check thoroughly all received scanned applications before an “acknowledgment receipt” can be made. Acknowledgement of receipt of application will be sent once the application is encoded in the consulate’s ePassport system, which is done only after office hours. To give time for the preparation of the pre-processed applications, follow up over the phone or email inquiry as to the receipt of the applications will not be entertained.

For proper management, it is requested that applicants should not resend several times their respective applications and should send only one application per email transmission with the applicant’s name in the subject field. If you did not receive an acknowledgement from the said email address, it will not be included on the list. Applicants are requested to wait for their respective acknowledgment receipt and check the list of pre-processed applications and their corresponding date/time slot for consular services, which will be posted in the consulate’s website, philippinesguam.org, and Facebook account on or before May 7, 2014.

To avoid delays in the processing of applications, applicants are advised not to fold copies of the requirements. Please be informed, however, that priority will be given to senior citizens (65 years old and above) and persons with disabilities.

* Requirements for lost Philippine passport

1. Accomplished ePassport Application form

2. Photocopy of data page of passport (if available)

3. Photocopy of birth certificate

4. Police Report

5. Notarized Affidavit of Loss ($25)

6. Original passport

7. Payment of Lost Passport Fee: $90 (green/maroon) or $150 (ePassport)

B. FOR CIVIL REGISTRY AND NOTARIAL SERVICES

All applications for civil registry and notarial services will be processed on a first-come, first-served basis. Applicants are advised to prepare the originals and copies of the required documents as indicated below:

* Report of Birth ($25)

1. Original Birth Certificate + five copies

2. If applicable, Original Marriage Certificate of Parents + five copies

3. Valid Passport of Parents + five copies; If Passport is expired, valid government issued ID + five copies

4. U.S. Naturalization Certificate of Parent + five copies, if parent is already a U.S. Citizen at the time of application

5. Affidavit of Delayed Registration of Birth ($25), if filed more than one month from birth of child

6. For Illegitimate children using surname of father, Affidavit to Use Surname of Father ($25)

7. Duly accomplished ROB Form, signed in front of consular officer

* Report of Marriage ($25)

1. Original Marriage Certificate + five copies

2. Original Birth Certificate of Filipino spouse + five copies

3. Valid Passports of both parties + five copies each; if passport is expired, valid government issued ID + five copies

4. 2×2 photo (white background) of each of the spouses

5. For parties with previous marriage/s:

a. For U.S. citizens, Final Decree of Divorce + five copies
b. For Filipino citizens, Annotated Marriage Certificate from NSO showing details of annulment + five copies or judgment of Philippine court recognizing final decree of divorce issued by a foreign court
c. For widows/widower, death certificate of deceased spouse

6. Affidavit of Delayed Registration of Marriage ($25), if filed more than one month from celebration of marriage

7. For those whose spouses are non-Filipinos, duly accomplished CFO form

8. Duly accomplished ROM Form signed in front of consular officer

* Report of Death ($25)

1. Original Death Certificate + copies

2. Duly accomplished ROD Form

* Acknowledgements (SPA, General Power of Attorney, Contracts), Jurat (Affidavits)

1. Valid Passport or government issued ID (i.e., driver’s license) of signatory + copy

2. Must sign document in front of consular officer

3. Payment of fee: $25

* For NBI Clearance Application ($25)

1. Completely accomplished NBI Form (to be provided by the Philippine Consulate Team). Said form must be brought to the local Police Station for fingerprinting. Applicants must ensure that the police officer taking the fingerprints must sign in the “Impressions Taken By” field.

2. One 2×2 photo (to be pasted on the form)

3. Original Valid Passport and two (2) copies (Data and Signature Page of the passport)

* Authentication/Verification of Employment Contracts

1. Job Order
Attachments: USCIS Approval, Company License, Passport of Signatory (Employer)

2. Special Power of Attorney
Attachments: Company License, Passport of Signatory (Employer)

3. Recruitment Agreement
Attachments: Company License, Passport of Signatory (Employer)

4. Individual or Group Employment Contract
Attachments: Company License, Passport of Signatory (Employer)

Note that all applicants for the above services will be required to photocopy the signed documents after processing.
