PCG to hold consular outreach on Saipan, Tinian

The Philippine Consulate General in Agana, Guam, will conduct mobile consular services on Saipan and Tinian this month.
The Saipan mission will take place on Sept. 23 to 25, from 9am to 5pm, at the Mt. Carmel Social Hall.
The outreach on Tinian will take place on Sept. 26, from 12pm to 5pm, and Sept. 27, from 9am to 5pm, both times at the Tinian Dynasty Hotel and Casino.
All of those interested in availing of consular services are advised to prepare the following requirements:

A. PASSPORT RENEWALS FOR SAIPAN ONLY
• Due to the capacity limits of our passport processing equipment, we can only process up to 360 applications over the three-day mission from Sept. 23 to 25;
• Please note that only the 360 applicants who had pre-registered during the last consular mission in May 2015 will be processed during this mission;
• The 360 applicants on the official list are each assigned a specific date and time slot;
• Make sure that you appear during your scheduled processing time, otherwise you may have to wait until a slot opens up;
• Those who need to renew their passports and are not on the official list of 360 applicants to be processed for this mission can still come to pre-register and submit the necessary requirements;
• Those who pre-register during this September mission will have their applications processed during the next consular mission;
• Per Department guidelines, the Consular Team may extend the validity of expired passports of those who have urgent need for a valid passport;
• The Consulate General is well aware of the issues faced by our nationals who cannot renew their passports and we are working on solutions to alleviate their situation;
• The official list of those who have appointments will be announced on Sept. 21, 2015;

B. PASSPORT RENEWAL FOR TINIAN ONLY
• Applicants for the passport renewal will be processed on a first-come, first-served basis based on the listed names submitted by Rod Cabarles to the Consulate General. Applicants are advised to prepare the copies of the required documents for the renewal of passport as indicated below.

Requirements for renewal of GREEN Philippine passport:
1. Accomplished ePassport Application form
2. Photocopy of data page of passport
3. Original passport
4. Payment of ePassport Fee ($60)

Requirements for renewal of BROWN/MAROON Philippine passport
1. Accomplished ePassport Application form
2. Photocopy of data page of passport
3. Photocopy of birth certificate
4. Original passport
5. Payment of ePassport Fee ($60)

Requirements for LOST Philippine passport
1. Accomplished ePassport Application form
2. Photocopy of data page of passport (if available)
3. Birth certificate issued by the Philippine Statistics Authority (PSA)
4. Police Report
5. Notarized Affidavit of Loss ($25)
6. Payment of Lost Passport Fee: $90 (Green/Maroon) or $150 (ePassport)

C. REGISTRATION FOR OVERSEAS VOTING FOR THE MAY 9, 2016 PHILIPPINE ELECTIONS
All citizens of the Philippines who are abroad, at least 18 years of age on the day of the elections (May 9, 2016), and who are registered overseas voters, may vote for president, vice president, senators and party-list representatives.
Personal appearance is necessary as the voter registration process requires the intake of a person’s biometric data. Applicants should present proof of Philippine citizenship, such as a valid Philippine passport or NSO-issued certificate of birth.

D. FOR CIVIL REGISTRY AND NOTARIAL SERVICES
All applications for civil registry and notarial services will be processed on a first-come, first-served basis. Applicants are advised to prepare the originals and copies of the required documents as indicated below:

Report of Birth ($25)
1. Original Birth Certificate plus five copies
2. If applicable, Original Marriage Certificate of Parents plus five copies
3. Valid Passport of Parents plus five copies; If Passport is expired, valid government issued ID plus five copies
4. US Naturalization Certificate of Parent plus five copies, if parent is already a US Citizen at the time of application
5. Affidavit of Delayed Registration of Birth ($25), if filed more than one (1) month from birth of child
6. For Illegitimate children using surname of father, Affidavit to Use Surname of Father ($25)
7. Duly accomplished ROB Form, signed in front of consular officer

Report of Marriage ($25)
1. Original Marriage Certificate plus five copies
2. Original Birth Certificate of Filipino spouse plus five copies
3. Valid Passports of both parties plus five copies each; if passport is expired, valid government issued ID plus five copies
4. 2×2 photo (white background) of each of the spouses
5. For parties with previous marriage/s:
a. For US citizens, Final Decree of Divorce plus five copies
b. For Filipino citizens, Annotated Marriage Certificate from NSO showing details of Annulment plus five copies OR Judgment of Philippine Court Recognizing Final Decree of Divorce issued by a foreign court
c. For widows/widower, Death Certificate of deceased spouse
6. Affidavit of Delayed Registration of Marriage ($25), if filed more than one (1) month from celebration of marriage
7. For those whose spouses are non-Filipinos, duly accomplished CFO form
8. Duly accomplished ROM Form signed in front of consular officer
Report of Death ($25)
1. Original Death Certificate plus copies
2. Duly accomplished ROD Form
Acknowledgements (SPA, General Power of Attorney, Contracts), Jurat (Affidavits)
1. Valid Passport or government issued ID (i.e., driver’s license) of signatory plus copy
2. Must sign document in front of consular officer
3. Payment of $25 fee

For NBI Clearance Application ($25)
1. Completely accomplished NBI Form (to be provided by the Philippine Consulate Team). Said form must be brought to the local Police Station for fingerprinting. Applicants must ensure that the police officer taking the fingerprints must sign in the “Impressions Taken By” field.
2. One 2×2 photo (to be pasted on the form)
3. Original Valid Passport and two (2) copies (Data and Signature Page of the passport)
