Notice to all Prior Service beneficiaries
All Prior Service Trust Fund beneficiaries whose monthly benefit checks are allotted to Bank of Guam, Bank of Hawaii, Back of Saipan, First Hawaiian Bank, Forcht Bank-(Somerset KY), and Wells Fargo Bank-(Lynnwood, WA) are being asked to respond to a questionnaire in order to continue receiving their benefits.
According to Jerold F. Facey, adviser of the Prior Service Trust Fund, in order to fulfill the Department of the Interior’s Office of Insular Affairs’ requirement to provide the continuation of funding to Prior Service and to confirm the continuing eligibility of its beneficiaries, you are required to complete a questionnaire and return it to us within 30 days upon receipt. Failure to comply with this requirement may result in the suspension of benefits.
“Please stop by at our office to pick up a questionnaire or call to inform us of your Post Office mailing address so we can mail it to you. Our office’s physical location is at Quarters No. 1201, Isleta Court, Capital Hill, Saipan, MP 96950. If you have any questions, call our office at (670) 322-9753 from 8am to 12pm and from 1pmto 4:30pm Tuesday, Wednesday, and Thursday.” (PSTF)
