Financial assistance to dialysis patients

Share

The American Kidney Fund has opened its Disaster Relief Program to dialysis patients in the CNMI.

The Disaster Relief Program provides financial assistance for up to $125 for each dialysis patient that was affected by Super Typhoon Yutu.

To request for financial assistance, eligible dialysis patients must complete an application form. Completed requests should be emailed to patientservice@kidneyfund.org or faxed to (301) 881-0898. For questions, you can contact AFK at 1-800-795-3226.

If you are a dialysis patient at CHCC and need help filling out the application forms, see its renal social workers, Chrislaine Pangelinan or Bridget Mahoney at the Dialysis Unit or call them at (670) 236-8303. (PR)
