DUA application submission deadline extended for CNMI

The CNMI Department of Labor announced it has re-extended the application submission deadline for Disaster Unemployment Assistance (DUA) for Typhoon Mangkhut (FEMA-4396-DR-MP) and Super Typhoon Yutu (FEMA-4404-DR-MP). The new deadline to apply for DUA is Feb. 14, 2019.

The DUA program provides payment relief and reemployment assistance to individuals who are unemployed as a direct result of a major disaster.

HOW AND WHERE TO APPLY

Claimants may apply for DUA either in person, by mail, or by phone. DUA applications and other forms may be obtained at either of the following location(s):

SAIPAN: Bldg. 1334, Ascencion Drive, Capitol Hill, Saipan, MP 96950

ROTA: Mayor’s Office, Tatachog, Rota, MP 96951

TINIAN: Edu’s Plaza, 1st Floor, San Jose Village, Tinian, MP 96952

Individuals applying by phone should have their social security number available when calling, along with the name and address of their last employer or prospective employer of record or basic information regarding their business, if self-employed. Phone applications can be made by calling any of the following numbers Monday through Friday between 9am through 3pm.

ROTA: 670-532-9479

Individuals who willfully conceal material facts or knowingly making false statement(s) to obtain DUA to which they are not entitled will be required to repay all benefits fraudulently collected and may be subject to prosecution under 18 U.S.C. 1001.
