Incoming 6th graders (Registration on June 20-June 24, 2016)
• Registration forms for SY ’16-‘17
• Birth certificate
• Update of school accident insurance or proof of health insurance coverage
• Updated health clearance certificate
• Parents/guardians and emergency contact information photo IDs (Passport, driver’s license, mayor’s ID, etc.)
• Copy of 5th grade completion
(Note: Pending submission of registration form and requirements will not be included in the Roster for School Year 2016-2017)
Ongoing 7th and 8th graders (Registration on June 27 to July 8, 2016)
