The Department of Community and Cultural Affairs’ Nutrition Assistance Program is calling on clients who suffered loss of food stamps to report to respective NAP offices for replacement.

NAP offices will begin accepting reports today until the following Friday, May 29.

“We would like to inform all current NAP households who have suffered a loss of food stamp coupons or food they purchased with food stamp coupons was destroyed due to Typhoon Dolphin to visit to the NAP office and request for a replacement,” NAP administrator Walter Macaranas said.

All current NAP recipient households affected by Typhoon Dolphin may qualify for replacement coupons, in accordance with the NAP Manual of Operations.

A statement of declaration will be required from the adult head of household declaring their description of loss and its value. Forms are available at the NAP offices. Only the adult head of household or their authorized representative may fill out the form. Forms will not be released and must be filled out at the NAP office.

Households are also required to bring their NAP I.D. card and a valid photo I.D. card.

“Households should be aware that they are signing the statement of declaration under penalty of perjury. After filling out the statement of declaration, their information will be verified by answering a set of questions from a NAP or DCCA official,” added Macaranas.

The household’s eligibility and information will then be reviewed and submitted for approval for a replacement.

“After the May 29, 2015 deadline, the NAP office will compile all requests eligible for replacement on Saipan, Tinian, and Rota and submit a detailed listing to the regional office in San Francisco for approval to release the replacements,” said Macanas.

Saipan households must visit the NAP office in As Lito; for Tinian, the Tinian DCCA office at the San Jose Gym; and for Rota households, the Rota DCCA office at the Rota Court House in Sinapalo. Offices are open from 7:30am to 4:30 pm.

For more information, contact the NAP Saipan office at 664-2800, the Tinian DCCA office (Marla Sablan) at 433-9330, or the Rota DCCA office (Donna Ogo) at 532-9351.
