TES registration schedule

Tanapag Elementary School will start registering returning students, new students and kindergarten students on May 12, 2008 through May 30, 2008 from 8am to 4pm at the main office.

Registration Requirements:

1. Registration Form

2. Child must be 5 years old by August 31, 2008. (For Kindergarten)

3. Withdrawal/Transfer Form from previous school (For transferring students)

4. Birth Certificate

5. School Entrance Health Certificate

6. Copy of updated Health Insurance/Updated Medicaid

7. Map to Residence/Home

8. Copy of Power of Attorney or Guardianship (If applicable)
